Pay Your Bill by Phone

Call us at (480) 353-0446. Over the phone we accept payment by Mastercard, Visa, Discover, AMEX and debit cards are accepted.

Pay Your Bill by Postal Mail

Your statement has a form you can fill out to include a check or credit card. Simply mail the attached form back to our practice.
