Request Medical Records

We're dedicated to making your experience as a patient smooth, comfortable, and efficient. We've prepared this section to help you learn more about the administrative aspects of your care.  All requests for medical records take at least 5-7 business days to complete.  There is a charge for the release of medical records. 

To request a copy of your medical records to be sent to another physician or hospital, please complete our medical release form, listed below. The medical release form can then be hand-carried or mailed to your treating physician's office.
